The Digital Triage Nurse is a critical role on the Galileo care team, collaborating with a multidisciplinary team to ensure patients receive the highest quality care when they have a clinical concern. This remote, full-time overnight role involves triaging incoming patient communications, conducting phone assessments to determine the best path of care, and assisting in the development of processes for urgent care and virtual triage.
The Care Manager employs strategies to manage a member's health issues, resolving barriers to care. They ensure patient progress through continuous monitoring. This involves telephonic outreach to assess and support health, offering education, identifying resources, and helping remove barriers to achieving health and independence, while using a multidisciplinary team.
This is a part-time W2 position, working 16 hours a week. As an integral member of the Hazel team, you will make an impact by increasing equitable access of quality health care experiences for students and their families. You will provide virtual health care services to students in partner school districts and collaborate with a diverse care team.
